I reserve space in the Columbarium at
Highland Park United Methodist Church.

After completing this reservation, I will be sent an
Applications for Certificate of Right of Inurnment for signature.
The Application will contain
Rules, Policies and Regulations
of HPUMC governing the Columbarium. I
understand Columbarium space will be assigned
in consultation with the member
reserving space, in the order of reservations made. I
also understand I will be allowed to cancel my
reservation at any time prior to execution of the
Application, and will be refunded my deposit without
interest in the event of cancellation. I understand
this Reservation and a payment made through the
website are needed to complete my reservation.
