Child Development Program - Registration
The Child Development Program, HPUMC’s school for children ages 6 weeks through age five, will begin its registration for the 2012-2013 school year in January. We welcome church and community families to apply for one of the two programs available:
For children not currently enrolled:
Tuesday, January 3, 2012 - Reservations begin for tours – call 214-521-2600
Wednesday, January 4 - Registration materials available for pick up. Application and fee accepted by mail only
Friday, January 27 - Application deadline
All-day classes: M-F, 7:30 a.m. – 6:00 p.m.
Infants through Bridge to Kindergarten (age five by 9-1-12)
Half-day preschool classes: 8:45 – 11:45 a.m. (Days vary by age)
Transitional Twos (18 months by 4-30-12) through Bridge to Kindergarten (age five by 9-1-12)
We welcome all to consider joining us as we grow together in learning that “God loves me, and God loves everyone!”
The following forms must be completed and turned in by the first day of attendance:
- Registration form: Information on this form must be kept up-to-date at all times. This includes persons to contact in case of an emergency, Discipline and Guidance statement, and CDP Policies and Procedures agreement.
- Physician’s statement: This form must be completely filled out and signed by child’s physician within one calendar year of Sept. 2010.
- Immunization form: All immunizations must be up-to-date for the child’s age.
- Photo identification form: Photos of all persons listed on the authorization to pick up form must be on file in the office.
- Custody agreement: Children of families in which and visitation schedule parents are divorced or separated need to have these forms on file.
